Below is our copy of a Generic Consent Form template for GDPR.
This can be used to build a customer list so you can e-commerce or m-commerce your potential customers AND most importantly have their permission to do so.
You can print some copies off internally to get you started or ask your local printer to print some off for you, either as an NCR pad or as loose sheets with your company details/logo and any additional information you would like to include.
